If you have read The Chambers of Secrets, you should have known the reason. The phrase "I Am Lord Voldemort" can be used as an anagram to create "Tom Marvalo Riddle".
The book also explained that Voldemort wanted to distance himself from his father Rom Riddle who was a muggle. This was the reason for the change of name.
